Eligibility Requirements
To be eligible for and to enroll in UA Medicare Part D Silver, you must be:
- Eligible to receive Medicare benefits under Part A or enrolled in Part B.
- Reside in the UA Medicare Part D Silver’s service area (continental United States)
Important Information
- You may only be enrolled in one Part D plan at a time.
- If you are enrolled in a Medicare Advantage (MA) plan, you may not enroll in UA Medicare Part D Silver unless you are a member of a Private Fee-for-Service (PFFS) or a Medical Savings Account MA plan (MSA).
Pharmacy Information
UA Medicare Part D Silver prescription drug plan has formed a network of pharmacies. You must use a network pharmacy to ensure you receive plan benefits. UA Medicare Part D Silver may not pay for your prescriptions if you use an out-of-network pharmacy, except in certain cases. The pharmacies in our network can change at any time. If you would like a Pharmacy Directory, you may call UA Medicare Part D Silver Customer Service at 866-299-3406 (TTY/TDD: 866-524-4170) to request the most current list.
Medicare Part D Financial Assistance
Beneficiaries interested in available Medicare Part D subsidies may contact UA Medicare Part D Silver Customer Service at 866-299-3406 (TTY/TDD: 866-524-4170), 800-MEDICARE/800-633-4227 (TTY/TDD: 877-486-2048), your state Medicaid office, or local Social Security Administration office.
If you have qualified for additional financial assistance for your Medicare prescription drug plan costs, the amount of your premium and cost at the pharmacy will be less. Once you have enrolled in a UA Medicare Part D Silver prescription drug plan, Medicare will tell us how much assistance you are receiving. We will then send you information on the amount you will pay. If you are not receiving this additional assistance, you should contact 800-MEDICARE/800-633-4227 (TTY/TDD: 877-486-2048), your state Medicaid office, or local Social Security Administration office to see if you qualify.
Medicare Supplement (Medigap) Policies with Part D
If you have a Medicare Supplement (Medigap) policy that includes prescription drug coverage, you must contact your Medigap provider to let them know that you have joined a Medicare prescription drug plan. If you decide to keep your Medigap policy, your Medigap provider will remove the prescription drug coverage portion of your policy and adjust your premium. Under certain circumstances, you can also buy a different Medigap policy without prescription drug coverage sold by your Medigap provider. Your Medigap provider cannot charge you more based on any past or present health conditions. Call your Medigap provider for details.
